Cancer Treatment Approaches

Cancer treatment is no longer one size fits all. Modern cancer care increasingly depends on the biology of the individual tumor, the stage of disease, the molecular profile, prior treatments, the patient’s overall health, and the goals of care.

Dr. Dean Silver, MD, MD (H), provides cancer education to help patients and families understand the major categories of cancer treatment, how they work, what testing may help guide treatment decisions, and how conventional and integrative approaches may fit together.

Understanding the Cancer Before Choosing Treatment
The diagnosis is only the beginning.
Important information may include pathology, tumor type, grade, stage, hormone receptor status, HER2 status, genetic mutations, molecular pathways, biomarkers, PD-L1 expression, microsatellite instability, mismatch repair status, tumor mutational burden, and other findings from molecular testing.
Next generation sequencing may identify mutations or pathways that can help explain why a cancer is growing and whether targeted therapies may be relevant.
Blood based testing, including circulating tumor DNA and circulating tumor cells, may also provide useful information in selected situations. These tests can sometimes help with monitoring, molecular profiling, and treatment response, but they do not replace tissue pathology, biopsy, or standard imaging.

Surgery
Surgery remains an important treatment for many localized solid tumors.
It may be used to remove the primary tumor, obtain tissue for diagnosis and molecular testing, reduce tumor burden, improve local control, or relieve symptoms.
The role of surgery depends on the cancer type, location, stage, extent of spread, and whether the tumor can be removed safely.

Radiation Therapy
Radiation therapy damages cancer cell DNA and may be used to treat localized tumors, metastatic lesions, microscopic residual disease, pain, or other complications.
Modern techniques may include stereotactic body radiation therapy, stereotactic radiosurgery, proton therapy, intensity modulated radiation therapy, and other targeted approaches.

Chemotherapy
Chemotherapy uses drugs that interfere with cancer cell growth and division.
Its effectiveness varies greatly depending on the type of cancer and the individual tumor.
Treatment selection may depend on pathology, stage, molecular findings, previous treatments, organ function, and expected benefit.
In some situations, chemotherapy sensitivity testing and functional testing are also discussed as additional educational tools, although these approaches do not replace standard oncologic decision making.

Targeted Therapy
Targeted therapies are designed to block specific proteins, receptors, mutations, or signaling pathways that cancer cells depend upon.
Examples include therapies directed toward HER2, EGFR, ALK, BRAF, KRAS, PARP pathways, PI3K pathways, CDK4 and CDK6, and many other molecular targets.
This is one reason molecular testing has become increasingly important. Identifying an actionable mutation can sometimes reveal treatment options that would not be apparent from the diagnosis alone.

Immunotherapy
Immunotherapy attempts to help the immune system recognize and attack cancer.
Checkpoint inhibitors are among the most widely used forms of immunotherapy. They may act on pathways involving PD-1, PD-L1, and CTLA-4.
Biomarkers such as PD-L1 expression, microsatellite instability, mismatch repair deficiency, and tumor mutational burden may help identify patients more likely to benefit from certain immunotherapies.
Immunotherapy can be effective in selected patients, but it can also cause immune related adverse effects because an activated immune system may attack normal tissues.

Hormone Therapy
Some cancers depend on hormones for growth.
Hormone therapy is especially important in hormone receptor positive breast cancer and prostate cancer.
Treatment may involve blocking hormone receptors, lowering hormone production, or interfering with signaling pathways that support hormone dependent cancer growth.

Antibody Drug Conjugates
Antibody drug conjugates combine a targeted antibody with a cancer killing drug.
The antibody recognizes a specific protein on the cancer cell and helps deliver the attached treatment toward the tumor.
This approach has become increasingly important in several cancers, including breast cancer and other tumors with identifiable molecular targets.

Repurposed Medications
Some medications originally developed for other diseases are being studied for potential anticancer effects.
Research has examined drugs that may influence inflammation, metabolism, autophagy, angiogenesis, cholesterol pathways, mitochondrial function, immune signaling, and other cancer related pathways.
The quality of evidence varies widely. Some drugs have human clinical data, while others are supported mainly by laboratory studies, observational findings, or early clinical trials.
Repurposed medications should not replace established cancer treatment without careful discussion with the treating oncology team.

Nutrition and Metabolic Approaches
Cancer cells often have altered metabolism.
Nutrition, insulin signaling, glucose metabolism, inflammation, body composition, and overall metabolic health may influence the biological environment surrounding cancer.
There is no single diet that treats every cancer.
Educational discussions may include protein intake, carbohydrates, plant based nutrition, fats, fasting strategies, ketogenic approaches, micronutrients, and the relationship between nutrition and conventional cancer treatment.

Vitamins, Supplements, and Botanical Compounds
Vitamins, minerals, antioxidants, botanical compounds, medicinal mushrooms, and other supplements are commonly used by patients with cancer.
Some may affect inflammation, oxidative stress, metabolism, immune function, or cellular signaling.
Natural does not automatically mean safe.
Supplements may interact with chemotherapy, radiation, targeted therapy, hormone therapy, anticoagulants, or other medications. Their use should be reviewed carefully.

Integrative Cancer Therapies
Integrative oncology combines conventional cancer treatment with selected supportive or complementary strategies.
Depending on the individual case and the evidence available, discussions may include hyperbaric oxygen, cryotherapy, infrared therapies, metabolic strategies, immune support, exercise, nutrition, sleep optimization, stress reduction, and other supportive approaches.
Some of these therapies have established supportive roles. Others remain investigational.
The goal is not to replace oncology care. The goal is to help patients understand additional strategies that may support overall health while standard cancer treatment is being considered or administered.

Emerging Cancer Therapies
Cancer research continues to evolve rapidly.
Emerging areas include cancer vaccines, cellular therapies, CAR T cell therapy, natural killer cell therapies, tumor-infiltrating lymphocyte therapy, new antibody drug conjugates, radiopharmaceuticals, molecularly targeted drugs, artificial intelligence-assisted treatment selection, and new blood-based monitoring technologies.
Not every emerging therapy is appropriate for every patient.
Understanding the biology of the tumor remains essential.
